Elegibilty

Any patient in the office with an account balance of zero is eligible to join the plan. Dentist will determine if patient requires periodontal membership. This membership can only be used for care at Milwaukee Dental Arts.

Annual Cost

  • $400/year for child and for adults (with regular cleanings and no periodontal disease).
  • $660/year for patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.

  • Doctor exams (1-2 per year)

  • One emergency exam per year (if needed)
  • 15% off all other treatments (some exclusions may apply)

Exclusions

Please note, discounts do not apply to the following treatments or procedures:
  • Aligners
  • Whitening

1. Membership Type: CareClub is an in-house membership, not a dental insurance plan. It provides discounted rates for services exclusively at Milwaukee Dental Arts and is not valid at any other dental provider.

2. Non-Transferable: The membership is valid only for the individual enrolled and cannot be transferred to another person under any circumstances.

3. Full Payment Required: Full payment for the CareClub membership is due at the time of enrollment. No services under the membership will be rendered without complete payment.

4. Validity Period:  The CareClub membership is valid for one year from the date of payment. After this period, the membership will expire, and you will need to renew it to continue receiving membership benefits.

5. Member Responsibility: It is the member’s responsibility to schedule their appointments. Membership benefits are only available when appointments are scheduled, and services are rendered within the validity period. Refunds will not be provided for subscription dues under any circumstances, including failure to schedule and maintain appointments.

6. Membership Benefits: CareClub provides discounted rates for dental services, but it does not cover any additional out-of-pocket expenses, including those for third-party services such as specialist referrals.
 
7. Limitations: CareClub is not an insurance plan and does not fulfill any insurance obligations. It is designed to help you save on preventive and other dental treatments provided directly by Milwaukee Dental Arts.
 
8. Renewal: Renewal of the CareClub membership is not automatic. You must renew your membership each year to continue enjoying the benefits.
